0. 뇌척수액의 흐름

Lateral ventricle (생성) -> Foramen Monro -> Third Ventricle -> Aqueduct of Sylvius -> 4th ventricle -> one foramen magendie, two foramen Luschka -> subarachnoid space -> spinal subarachnoid space -> arachnoid villi to venous sinus  

1. 정의 

뇌척수액의 생성, 흐름, 흡수의 장애로 인해 생기는 신경학적 장애 (넓은 의미)

수두증(물뇌증, hydrocephalus)이란·뇌척수액의 생성과 흡수의 불균형에 의하여 뇌척수액이 축적되어 뇌실이 확장되고, 두개강내압 상승을 초래하는 병적 상태를 의미한다. (신경외과학, 4판, 대한신경외과학회)

1) Obstructive hydrocephalus: CSF pathway block으로 발생한다. 응급 상황이다. 

2) Non obstructive hydrocephalus: CSF overproduction(choroid plexus tumor) 또는 CSF absorption 장애로 인해 생긴다 (SAH)

* SAH 이후에 CSF drainage에 장애가 생기게 되는데, basal cistern이나 ventricle 내에  blood clot이 있고, arachnoid villi가 막히기 때문이다. SAH 이후에는 urgent CSF drainage를 요하는데, 이 경우 EVD, lumbar drainage를 시행할 수 있다. 장기적으로는 ventriculoperitoneal shunt 를 한다. 

2. 증상

acute IICP: nausea, vomiting, headache, mental deterioration. 아이의 경우 head size 증가

communicating hydro: gradual mental deterioration, cognitive impairment, lower extremity weakness, voiding problem, dementia

3. 진단

CT, MRI, 영아는 소노, ICP 측정 (요추천자, 뇌실천자)

4. 치료

acetazolamide, furosemide - CSF 생성 억제

surgical treatment - EVD, ventriculoperitoneal shunt

종양이나 출혈 등의 원인 치료

5. Normal pressure hydrocephalus (정상압 물뇌증)

뇌실은 확장되어있으나 압력은 정상인 물뇌증

원인: SAH, meningitis -> 저항의 증가 -> 뇌실의 확장 

증상: 보행장애, 인지장애, 배뇨장애

진단: CT, MRI에서 ventriculomegaly 발견

치료는 shunt operation 

6. 수두증의 영상의학적 소견 (중요한 순서대로) 

1) 제3뇌실의 확장, 특히 anterior and posterior recess (가장 중요)

2) temporal horn의 dilatation 

3) Narrowing of the mamillopontine distance

4) Narrowing of the ventricular angle

5) Widening of the frontal horn radium

6) Periventricular interstitial edema

7) Effacement of cortical sulci

dilatation of chiasmatic and infundibular recess of the third ventricle (stars)

제4뇌실도 확장된 것을 볼 수 있음
