c.c 손이 떨려요 51세 남환. V/S BP 130/76 PR 67 RR 11 BT 36.6

오른 손의 떨림으로 내원. (onset: 1YA). 텔레비전을 볼 때 손을 올려두면 떨음. 걸음 느려지고 목소리도 원래는 쩌렁쩌렁했었는데 바뀜. 손글씨도 작아진 편. 기억력은 좋음. Dizziness (+, 일어설 때), sweating(-)

Past history: 당뇨병 (6년) Family history: 아버지가 손을 많이 떠는 편.

P/E: 오른손 안정 떨림. 오른팔 톱니바퀴 경축. Rapid alternating test에서 우측 팔의 현저한 떨어짐.

케이스 설명

1. Parkinson’s disease

-진단(다음 중 2개 이상이며, tremor at rest와 bradykinesia중 하나는 반드시 있어야함)

tremor at rest, bradykinesia, rigidity, loss of postural reflexes, flexed posture of neck/trunk/limb, freezing

-원인: loss of melanin-containing neurons & presence of lewy bodies in substantia nigra.

Striatum에 도파민 감소

-증상: 목소리가 작아짐. 가면양 얼굴. Dysarthria. Micrographia. Resting tremor. Cogwheel rigidity. Gait

Disturbance(shuffling, freezing, 더 자주 넘어짐, 보폭이 작아지고 양 발 사이의 간격 좁아짐).

Postural instability.

-전신 증상:

GI) constipation

sensory symptoms) pain, paresthesia, numbness, hyposmia

GU) bladder problem, sexual dysfunction

Neuro) sleep fragmentation, RLS, RBD, depression, anxiety, delusions, hallucinations, dementia

Others) orthostatic hypotension, fatigue

-치료: levodopa, dopamine agonists, selective MAOB inhibitors(selegiline, rasagilie), anticholinergic agents,

NMDA antagonists

수술적 치료로 thalamotomy, pallidotomy, deep brain stimulation

2. Side effects of Levodopa

-peripheral: nausea/vomiting, cardiovascular dysfunction, 다리 부종

-Central: drowsiness, hallucination, mental change, impulse control disorder
